2. Complete Granum Grant Application:
- Cover Sheet
- Narratives
- Budget
- Facility Contract
3. Supplementals (support literature, biographies, videos/links, etc. to help screening committee understand project)

Copy of 501(c)(3) federal Determination Letter OR copy of most recent 990 form

Copy of minutes with approved Board resolution in support of Granum Grant request
