Devices & Technology Grant
Funder: Danny Did Foundation (Illinois, US)
Mission: The Danny Did Foundation strives to assist individuals and families living with seizures by providing monetary funding to gain access to seizure detection devices and technology.

- Devices may include: Sami Monitor, EpiMonitor, EpiWatch App, Inspyer App, Emfit Movement Monitor, and other seizure detection technologies
Focus Areas
- Applicants: Adults living with epilepsy seeking assistance, OR parent/legal guardian or primary caregiver for a person with epilepsy
- Geographic scope: All 50 states and 14 countries (foundation has provided support globally)
- Financial need: Foundation prioritizes those with demonstrated financial challenges
- Device user's neurologist required: Applicant must provide neurologist's name, hospital, and address
Eligibility
Funding Types
Two grant options available: 1. Hardship Grant: For those unable to make a purchase; applicant receives check and purchases device 2. Reimbursement Grant: For devices purchased within last 30 days; partial reimbursement considered upon receipt submission- No device has been proven to prevent epilepsy-related mortality (including SUDEP)
- Foundation does not direct, limit, participate in, or manage the purchase decision
- Many device resources are consumer products, not medical devices
- Foundation strongly recommends consultation with medical professionals and device manufacturers
- Foundation does not warrant any device, manufacturer, product, or treatment
- Foundation is not responsible for results and consequences of device use
Important Disclaimers
- Photo of person who will use device required before grant issued
- Receipt or proof of purchase required within 14 days of receiving grant check, or funds must be returned
- For EpiWatch App and EpiMonitor: prescription from doctor required
- For EpiWatch App and EpiMonitor: device user cannot share phone; parent/caregiver needs own smartphone to receive alerts

How to Apply
Application Process
1. Review Application Materials
- Review the Waiver and Release document before beginning
- Confirm eligibility requirements
2. Complete Online Application Form
- Submit via online form on foundation website
- Confirmation screen appears after successful submission
- Note: No immediate automated confirmation email is sent
3. Required Information
- Full name and contact information (email, phone, mailing address)
- Annual household income and number of family members in home
- Demographic information (optional: ethnicity, preferred language for interview)
4. Referral and Discovery
- How you heard about Danny Did Foundation (include referral name if applicable)
- How you learned about seizure detection devices
5. Device User Information
- Name of person who will use device/resource
- Relationship to applicant
- Date of birth
- Name of device/product seeking funding
6. Narrative Questions (minimum 100 words each)
- How would the device features meet the user's needs?
- Describe your financial challenges
- How would your family be impacted by receiving this grant? (be as specific as possible)
7. Medical Provider Information
- Device user's neurologist name and hospital
- Neurologist's complete address
8. Prior Assistance
- Have you previously received financial assistance from Danny Did or another organization for a device?
- If yes, describe the assistance and device name
- Are you currently seeking funding from other organizations?
9. Grant Type Selection
- Select HARDSHIP GRANT or REIMBURSEMENT GRANT
- Place initials to confirm understanding of selected grant type
10. Required Uploads and Agreements
- Upload photo of person who will use device (required before grant issued)
- Place initials confirming you understand EpiWatch/EpiMonitor phone requirements
- Place initials confirming understanding of prescription requirement for EpiWatch/EpiMonitor
- If seeking reimbursement: upload receipt(s) of device purchase (max 50 MB file size)
- If seeking hardship grant: submit receipt within 14 days of receiving check
- Upload prescription for EpiWatch App or EpiMonitor (max 50 MB file size)
11. Insurance and Demographic Questions
- Type of health care insurance plan (optional)
- Additional demographic questions (optional)
12. Agreement and Initials
- Place initials confirming all statements and requirements
- Agreement to return funds if unable to proceed with purchase (hardship grants)
- Agreement to provide receipts within 14 days (hardship grants)

- Spanish-speaking translator available if needed
