Chartercare Wellness/Obesity Prevention/Nutrition Education Grant

The Chartercare Wellness/Obesity Prevention/Nutrition Education Grant supports health initiatives in Rhode Island, offering up to $25,000 for programs aimed at enhancing wellness, nutrition education, and obesity prevention, particularly for disadvantaged groups. Eligible applicants are 501(c)(3) nonprofits in good standing. Proposals should detail program scope, patient impact, care improvement strategies, timely access to services, and outcome tracking methods. Preference is given to innovative solutions addressing critical needs.

Chartercare Wellness/Obesity Prevention/Nutrition Education Grant
Applications are now being accepted for proposals to provide structured programs to identify, treat, and track the effectiveness of wellness programs, nutrition education, diabetes and obesity prevention services, particularly among disadvantaged populations. Proposals up to $25,000 will be considered; and must include the following:
Scope of the program;Number of patients it will help,How the continuum of care will be improved;How access to effective programs and assistance will be achieved in a timely mannerHow patient progress will be monitored and tracked, ie: how outcomes will be measuredIf this proposal is not funded, how will the organization address this need?

Eligibility

Applicants must be a 501©3 Rhode Island nonprofit in good financial standing; grants are awarded to organizations, not individuals.

Ineligibility

Incomplete proposals will not be considered.
