Chartercare Foundation Our grants benefit health and wellness programs throughout Rhode Island. Senior Wellness & Safety Applications for program grants to provide structured quality of life programs (cultural, music, & the arts) as well as services for elderly populations, including increased awareness and training regarding proper safety measures, tips, and tools. Programs should encourage participation by health care professionals as well as family members with special emphasis on population gaps in underserved communities. Proposals must address the following: the scope of the program; number of patients it will help, how access to the program will be achieved; how patient progress will be monitored and tracked; and how outcomes will be measured.Eligibility
