    - Medical conditions, chronic ailments, allergic reactions, disabilities, or school IEP accommodations

    2. Parent/Guardian Contact Information
    - Name, relationship to participant
    - Address
    - Mobile and work phone numbers
    - Email address
    - Authorization for pickup/drop-off

    3. Emergency Information
    - Emergency contact person (other than parent/guardian)
    - Authorized pickup persons (up to 4 additional people)

    4. Essay Statements
    - Written by applicant: Statement describing background in theatre, music, or dance and/or hobbies
    - Written by applicant: Statement explaining what they love about musical theatre and why they want to participate
    - Written by parent/guardian (for scholarship applicants): Statement defining financial circumstances and how the child would benefit from attending

    5. Medical and Legal Releases
    - Release of Liability agreement
    - Medical Release authorization
    - Authorization to Reproduce Physical Likeness (photos/videos for promotional materials)
