Overview: Annual competitive grants program supporting Brevard County nonprofits working to improve community wellness and address health disparities.

Award Amount: Total of $360,000+ awarded to approximately 12 nonprofit organizations annually

Geographic Scope: Brevard County, Florida

Eligible Applicants:

Brevard County-based nonprofits

Organizations addressing identified health disparities

Community partners focused on wellness improvement

Focus Areas:

Community health disparities

Aging services

Youth services and prevention (homelessness, crime)

Healthcare navigation programs

Specialized health services (memory disorders, family caregiving)

Adaptive sports and recreation programs

Mobile health services

Selection Criteria:

Alignment with Community Health Needs Assessment priorities (conducted in partnership with Space Coast Health Foundation)

Demonstrated ability to produce measurable results

Enterprising approaches and expansive thinking

Impact on improving wellness and addressing health disparities

Evaluation Process:

Applications reviewed and approved by Community Health Improvement Committee

Committee comprised of Health First system leaders

Focus on measurable outcomes and life-saving impact
