FMC Foundation of Caring Grant Program

Funder: Florida Medical Clinic Foundation of Caring, Inc.

Mission Alignment: The foundation funds projects that align with FMCFOC's mission, vision and values and address specific, identified, and prioritized community needs.

    Geographic Scope

  • Service Areas: Hillsborough and Pasco Counties, Florida
  • Programs must serve individuals in Florida Medical Clinic's geographic or service areas

    Funding Details

  • Minimum Award: $1,000
  • Maximum Commitment: No more than a year commitment
  • Award Frequency: Once per year, awarded in December at a Grant Award Reception
  • Timing: Notifications sent October/November; check presentations November/December

Eligibility Requirements

1. Must be an IRS recognized 501(c)(3) charity 2. Must be currently registered with the Department of Agriculture to solicit funds in the state of Florida 3. Cannot have received a grant award from FMCFOC within one year of current submission (one-year waiting period between awards) 4. Must demonstrate strong management skills and sound leadership qualities 5. Must have been conducting substantial business activity for at least 2 years 6. Project must align with FMCFOC's mission, vision and values 7. Project must address a specific, identified, and prioritized need in the community 8. Project must be creative and have positive impact on in-need and at-risk individuals 9. Project must leverage dollars and/or people power 10. Project must identify measurable objectives 11. FMCFOC's grant must play an important role in project success 12. Previously awarded organizations must have submitted Impact Reports in a timely fashion

    Grant Cycle Timeline

  • Phase I - Application Acceptance: March 1st – June 30th
  • Phase II - Site Visits: July board meeting to review; August-October site visits
  • Phase III - Final Determination: October special board meeting; October/November notifications; November/December check presentations
  • Phase IV - Impact Reporting: 6-month report due June 1st of following year; 12-month report due December 1st of following year

Purposes NOT Funded

1. Start-up costs for new organizations/seed money 2. Capital campaigns for physical assets 3. Sponsorship or tickets for fundraising events or advertising space (unless in recognition of awarded grants) 4. Legislative lobbying or political campaigns 5. Experimental medical research or medical trials 6. Projects serving exclusively religious/sectarian purposes without community benefit to in-need/at-risk individuals 7. Loans, assistance, or scholarships to individuals 8. Public and private schools (except Title One and special needs schools) 9. Recreation, athletic, or sports team programs (except those serving individuals with special needs) 10. Agencies providing benefits to FMCFOC officers or employees in exchange for funding 11. General salaries or operating expenses (unless program-specific and directly benefit individuals served) 12. Non-service or non-therapy related animal organizations

    Applicants must demonstrate:
    1. Current IRS 501(c)(3) recognition

    3. No grant awards from FMCFOC within the past year

    6. Alignment with FMCFOC mission, vision, and values

    9. Documentation of leveraged dollars and/or people power
    10. Measurable objectives
    11. Evidence that FMCFOC grant plays important role
    12. Timely submission of impact reports (if previously funded)

    #### Post-Award Reporting Requirements

  • 6-Month Impact Report: Due June 1st of the year following award (not required if all funding spent by that date)

  • 12-Month Final Report: Due December 1st of the year following award (required if funding not fully spent to explain remaining fund utilization)
