Grant Guidelines of Unrestricted Funds

Mission Alignment: Organization must be in alignment with Nason Foundation's Mission: "To promote the health and vitality of our community through financial support and education."
Eligibility Requirements
1. Organization must be located in or serving the service area of Nason Hospital 2. Must be a 501(c)(3) organization or organization that qualifies for a deductible contribution using IRS rules 3. Must be a qualified organization that meets the 50% limit organization rules- One grant per organization per 12-month period (to conserve limited resources)
- Applications approved on quarterly basis by Nason Foundation Board
- Applicants notified within 4 weeks after Board decision
Application Restrictions
- A letter describing how funds were utilized and/or a site visit may be required

Application Requirements
Submit the following items:
1. Grant Application Cover Sheet (download from website)
2. Copy of current IRS determination letter indicating 501(c)(3) tax-exemption status OR letter from organization stating it meets the 50% limit organization rules
3. Most recent audited annual financial statement
4. Narrative of the project (1 to 5 pages)
5. Detailed budget for the project
6. List of the Board of Directors
7. Letters of support (OPTIONAL) - substantiate the need for the project and collaboration with other organizations
