Background: Essentia Health has a more than 20-year history of investing in communities across its service area. In FY23, Essentia Health contributed more than $1.7 million dollars to 140 non-profit organizations in the communities served.

Priority Focus Areas

Essentia Health prioritizes contributions to non-profit organizations working to address social determinants of health in four key priority areas:

1. Economic Stability (Housing, Workforce, Childcare, Healthy Food)
- Affordable housing projects
- Career solutions and subsidized employment services
- Childcare support
- Access to fresh, healthy foods

2. Access to Education and Healthy Youth Development (Reduction of adverse childhood experiences (ACES) and asset building for youth)
- Community and youth centers
- Technology hubs for students
- Youth mentorship programming
- After-school educational support and summer camps

3. Access to Right Time, Right Place Healthcare (Increase access to comprehensive, high-quality healthcare services)
- Free or subsidized youth dental services
- Health and wellness centers

4. Neighborhood Improvements that Promote Health and Safety (neighborhood and environmental changes)
- Park rehabilitation projects
- Community safety initiatives

  • CornerStone Community and Youth Center in Frazee
  • Technology Hub for students in Deer River
  • Youth mentorship programming with Mentor North
  • After-school educational support and summer camps for New American Youth in Fargo with South Sudanese Foundation
  • Free youth dental services with Just Kids Dental
  • Chum's Health and Wellness Center
  • City of Ashland East End Park Rehabilitation Project

Eligibility

  • Must be a non-profit organization
  • Organization must be focused on addressing social determinants of health in one of the four priority areas
  • Geographic focus on Minnesota, North Dakota, and Wisconsin communities served by Essentia Health
