Geographic Scope: Minnesota, North Dakota, and Wisconsin

Background: Essentia Health has a more than 20-year history of investing in communities across its service area. In FY23, Essentia Health contributed more than $1.7 million dollars to 140 non-profit organizations in the communities served.

Focus Areas: Projects and programs that improve health by addressing social determinants of health in four key priority areas:

• Economic Stability: Housing, workforce development, childcare, and healthy food access
• Access to Education and Healthy Youth Development: Reduction of adverse childhood experiences (ACEs) and asset building for youth
• Access to Right Time, Right Place Healthcare: Increase access to comprehensive, high-quality healthcare services
• Neighborhood Improvements that Promote Health and Safety: Neighborhood and environmental changes that promote health and safety

Eligibility: Non-profit organizations working to address social determinants of health

Application Process: Rolling basis review. Applicants should allow at least 90 days for a response due to high volume of applications.
