Note: Mini grants can only be accessed through a code given by PCHF staff. All organizations must contact the foundation to talk about your mini-grant idea before receiving the code. PCHF offers eligible organizations serving the greater Phoenixville area the opportunity to apply for a limited number of mini grants per fiscal year until the pool of funds is exhausted. Ride for Health Ride for Health is designed to help individuals in the Greater Phoenixville Area who have no other means of transportation to get to and from healthcare or human/social service-related appointments. These grants are to provide client-serving organizations with funds for transportation to and from certain healthcare and social service appointments. This grant will support internal Lyft Business or Uber Business dashboard rides, Lyft or Uber gift cars, and SEPTA transportation passes to help transport clients who qualify for Ride for Health.Eligibility

Organizations must have the following:Current 501(c)3 status (or fiscal sponsorship)Services provided to PCHF’s geographic area of interestDirectors and Officers insuranceOperating budget of less than $3 millionMust be a current annual grantee OR have received an annual grant within the last 2 years. Participating organizations must meet the following criteria: Organizations must be an invitee of the RFH grant initiative and have received the passcode from PCHF staff.Organizations may not have outstanding RFH funds. However, invited organizations may apply for more funding once funds are spent and any reports are completed.Participating partners must ensure all rides meet the following criteria: Anything healthcare OR human/social services related such as: Health Care Medical Eye exams Dental Mental health/counseling WIC Pharmacy Physical therapy Other healthcare related appointments Human/Social Services Applying for Welfare, Medical Assistance, SNAP, and other benefits Parole hearings Substance abuse treatment Job training Travel to a shelter for those experiencing domestic abuse or homelessness Other human/social services related appointments Participating partners must ensure all clients meet the following criteria:The Client must LIVE in one of the following townships or boroughs to participate:CharlestownCollegeville

Ineligibility
We do not reimburse for projects/programs that have already been funded or paid for by the applying organizations.Client’s ride must NOT be eligible for the Medical Assistance Transportation Program (MATP).Chester County residents aged 65 or older needing transportation to healthcare-related appointments are NOT eligible for the Transportation Grant funds as they are eligible for free transportation to healthcare appointments through ChescoConnect.Visit ChescoConnect’s website for more information.Exception: Phoenixville residents aged 65 or older that need same-day emergency medical appointments or surgical procedures that are outside the normal ChescoConnect hours.Clients must NOT be eligible for any other free transportation program or stipend.Focus Areas & Funding Uses
