Southwest Ohio T1D Community Grants

Breakthrough T1D Southwest Ohio is pleased to introduce its new micro-grant program to provide funding to our existing local community support groups and to assist with the development of new groups.

JDRF SWOH Community Outreach Grant Application

Existing T1D support groups that have been officially approved by the JDRF Southwest Ohio chapter are eligible to receive up to $250 for community events. Grants are made on an ongoing basis throughout the year, and there is no application deadline.

Eligibility

* Applicant must be affiliated with an officially approved JDRF SWOH Support Group and must be requesting funding for an event hosted by the support group.
* The event must support the goal of providing education, awareness, or community building for the T1D constituents of Southwest Ohio.
* If approved, promotion of the event must include the JDRF logo and the verbiage "Sponsored by JDRF Southwest Ohio".
* Application must be submitted at least 30 days prior to the event.
* For approved event expenses that are not paid directly by JDRF Southwest Ohio, detailed receipts must be provided to the chapter within 30 days of the event.
