Required Information

Organization Details:
1. Organization name and address (street address, city, state, ZIP code)
2. Website
3. Tax-exempt status documentation (501(c)(3) or exemption under another organization with evidence)
4. How you heard about SAR Charitable Foundation

Funding Request Information:
1. Amount of request
2. Date of request
3. Specific program/project/event for which funding is sought
4. How the request fits with SAR Charitable Foundation priorities
5. Goals, objectives, and problems/issues addressed
6. Percentage of funds going to payroll/admin costs vs. charitable purposes
7. Counties served
8. Organization's selection process for individuals entering the program or receiving aid
9. Number of individuals or families benefited per month and per year

Required Documents:
1. Financial documentation including:
- Most recent financial statement
- IRS Form 990
2. List of Board of Directors
3. One of the following (attach as file):
- Flyer describing organization mission, goals, and accomplishments
- Brochure
- Newsletter
- Annual report
4. If financial statement unavailable, provide written explanation
5. List of Board of Directors (if not included in flyer/brochure)

File Upload Requirements:

Deadline: August 31 (applications accepted February 1 - August 31 annually)

Important: Incomplete applications will not be processed. Complete applications can be resubmitted before the August 31 deadline.
