• Home remodeling (focuses on needed repairs only)

• Homeowners in Oakridge-Westfir area

Application Process

Step 1: Complete Needs Assessment Form

• Homeowners download fillable PDF form from SVdP website
• Form requests information about home repair needs and homeowner eligibility

Step 2: Submit Application

• Email completed form to: andy.clay@svdp.us

Step 3: Program Manager Review

• Andy Clay reviews application for income eligibility and repair needs
