Overview

Patient Assistance Grant (PAG) Program

Status: Active; accepting applications through December 1st each year or until funds depleted

Geographic Scope: Global/Worldwide

Background: The PAG program has been part of DSF's advocacy efforts since 2009. It recognizes how difficult it can be for families to obtain necessary equipment and items not typically covered by private insurance or Medicaid.

Eligibility:

  • Open to all patients worldwide diagnosed with Dravet syndrome

  • Must be a member of the DSF Family Network

  • No income requirements (need-based, not income-based)

  • Applications may be submitted by: patient's family, healthcare provider, or social worker
  • Award Amount:

  • Up to $1,500 per year

  • Lifetime maximum: $5,000 per patient

  • Awards given on first-come, first-served basis

  • Each applicant may apply once per grant cycle
  • Covered Items:

  • Durable medical equipment (wheelchairs, orthotics, cooling vests, adaptive strollers/wagons, adaptive bikes)

  • Seizure helmets and hats

  • Seizure monitors

  • Sleep-safe pillows

  • Therapy tools and devices

  • Communication and learning devices (e.g., iPads)

  • Educational devices

  • Expenses related to starting or maintaining the ketogenic diet

  • Tools and supplies associated with the keto diet
  • Items NOT Covered:

  • Medical co-pays

  • Therapy costs/sessions

  • Respite assistance/care

  • Service animals

  • Medical travel
  • Application Period: January 1 - November 30 each year (or until funds depleted)

  • Submit through online application portal on DSF website

  • Maximum of 3 items per application (contact DSF if requesting more)
