Eligibility Overview

    Care Provider Requirements

  • Primary care provider must be a member of the Academy of Osseointegration
  • Funds are used for care and treatment of only the individual (patient) specified in the application
  • Funds should subsidize all or part of care needed by the patient
  • Commitment to complete therapy and 3-year follow-up is required when accepting grant funding
  • Patient should not incur any financial obligations
  • All team members must agree to relieve the patient of any financial responsibilities
  • Diagnosis and full treatment plan (including restorative plan) must be submitted for evaluation
  • Patient anonymity can be assured in the proposal
  • Documentation required: pre- and post-operative projection-quality slides and radiographs
  • Patients may be asked to agree to be photographed or filmed for promotional activities (patient release form required)
  • Three years of follow-up reporting to the Osseointegration Foundation is required (submitted annually)

Patient Eligibility Requirements

Patients must meet eligibility requirements in each category:

Physical Disability

  • Must have a disability amenable to implant therapy

  • Implant treatment would significantly improve the patient's dental deficit and quality of life
  • Economic Status

  • Documentation must provide evidence of inability to receive needed implant therapy due to economic constraints

  • Economic hardship is not the primary or sole criterion for awarding a grant
  • Emotional Well-being

  • Benefits of psychological or emotional improvement from implant therapy will be considered in award selection
    • Corporate Partner Participation

    • Program requires involvement of Osseointegration Foundation's commercial partners for service and/or hardware
    • All materials should be donated by the specified company
    • Grant applicants must specify particular products (including manufacturer names) at time of application
    • Applicants should contact commercial entities to obtain donations for required materials
    • If applicant lacks specific company contact, the OF will provide list of potential suppliers
    • Both care providers and corporate partners receive narrative recognition at official Academy meetings, JOMI, and Osseointegration Foundation Report

      Funding Restrictions

    • Grant does not restrict how funds are allocated by service provider, as long as entire grant is spent directly on patient care and treatment in osseointegration domain
    • Under no circumstances will the patient receiving care through this grant program be charged for any services rendered

    Application Process
