OF Charitable Grant Program

Purpose: Provides financial assistance to improve the quality of life for patients diagnosed with a lifetime dental deficit or who have suffered from severe dental trauma and are unable to receive needed dental therapy due to economic restraints.

Geographic Scope: Likely North America (AO members, based in Illinois, US)

Eligibility Overview

The OF Charitable Grant is only available to AO member dental professionals. Patients cannot apply for this grant directly.

Care Provider Requirements

• Primary care provider must be a member of the Academy of Osseointegration
• Funds are used for care and treatment of only the individual (patient) specified in the application
• Funds should subsidize all or part of care needed by the patient
• Commitment to complete therapy and 3-year follow-up is required when accepting grant funding
• Patient should not incur any financial obligations
• All team members must agree to relieve the patient of any financial responsibilities
• Diagnosis and full treatment plan (including restorative plan) must be submitted for evaluation
• Patient anonymity can be assured in the proposal
• Documentation required: pre- and post-operative projection-quality slides and radiographs
• Patients may be asked to agree to be photographed or filmed for promotional activities (patient release form required)
• Three years of follow-up reporting to the Osseointegration Foundation is required (submitted annually)

Patient Eligibility Requirements

Patients must meet eligibility requirements in each category:

Physical Disability

• Must have a disability amenable to implant therapy
• Implant treatment would significantly improve the patient's dental deficit and quality of life

Economic Status

• Documentation must provide evidence of inability to receive needed implant therapy due to economic constraints
• Economic hardship is not the primary or sole criterion for awarding a grant

Emotional Well-being

• Benefits of psychological or emotional improvement from implant therapy will be considered in award selection

Corporate Partner Participation

• Program requires involvement of Osseointegration Foundation's commercial partners for service and/or hardware
• All materials should be donated by the specified company
• Grant applicants must specify particular products (including manufacturer names) at time of application
• Applicants should contact commercial entities to obtain donations for required materials
• If applicant lacks specific company contact, the OF will provide list of potential suppliers
• Both care providers and corporate partners receive narrative recognition at official Academy meetings, JOMI, and Osseointegration Foundation Report

Funding Restrictions

• Grant does not restrict how funds are allocated by service provider, as long as entire grant is spent directly on patient care and treatment in osseointegration domain
• Under no circumstances will the patient receiving care through this grant program be charged for any services rendered

Application Process

1. Eligibility Confirmation

   • Confirm patient meets physical disability, economic status, and emotional well-being criteria
2. Corporate Partner Coordination
   • Contact commercial entities to obtain donations for required materials
   • Specify particular products (including manufacturer names) in application
   • Alternative: Request list of potential suppliers from OF if no specific company contact available
3. Required Application Materials
   • Complete diagnosis and full treatment plan (including restorative plan) for the patient
   • Pre-operative documentation: projection-quality slides and radiographs
   • Evidence of patient's inability to receive needed implant therapy due to economic constraints
   • Patient anonymity can be assured - patient identity is irrelevant to evaluation
   • List of specified products and manufacturers to be used
4. Patient Documentation
   • Patient release form (required if patient agrees to be photographed or filmed for promotional activities)
5. Post-Award Obligations
   • Submit pre- and post-operative documentation (projection-quality slides and radiographs) for each grant-funded case
   • Provide three years of follow-up to the Osseointegration Foundation
   • Follow-up should be submitted to OF on an annual basis
   • Complete therapy commitment and 3-year follow-up are required
