Ministry Grant Program
Funder: Nebraska United Methodist Foundation
Geographic Scope: Nebraska United Methodist churches and affiliated churches and agencies of the Great Plains United Methodist Conference
Purpose: To enhance and expand ministries of organizations. The ultimate goal is to provide assistance, whether a one-time grant or a grant that will help a ministry become self-sustaining.

3. Required Attachments
- a) Narrative with details about the ministry including purpose, plan, and significance of program
- b) Mission and/or Vision Statement
- c) Timeline and ministry/program budget (including all sources of funding)
4. Application Questions
- What is the amount you are requesting from the Foundation?
- Do you have other funding to support your initiative? (Yes/No)
- Have you received a ministry grant from the Foundation within the last five years? (Yes/No)
- If yes, list all grants received and total amount
- Was the entire grant used? (Yes/No)
- How was the grant used? Include details of the use of those funds and how the grant enhanced the outreach of your church/ministry (attach additional pages if necessary; photos and stories are appreciated)
Important Note: The Foundation continues to honor donor intent and values the results of the Ministry Grant program. Lack of communication on the results of prior years' projects could impact future grant requests.
