    2. Download and print the Agreement Form after online submission
    3. Sign the Agreement Form in blue ink
    4. Obtain 2 letters of recommendation/reference
    5. Mail the signed agreement form and 2 letters of recommendation to the address listed on the agreement form by the deadline
    6. Receive notification shortly after the Board Meeting regarding application status

    2. Share how you will continue to fund this project once the startup funds from Life Line Homes are exhausted
