Grant Application Process

#### Deadlines

• Online applications: Submit by January 30th for February Board meeting consideration OR July 14th for August Board meeting consideration
• Signed agreement form & references: Must be postmarked by January 31st or July 15th
• Late applications will not be considered

#### Step-by-Step Application Process

1. Complete the online grant application form on the website (all areas must be completed)
2. Download and print the Agreement Form after online submission
3. Sign the Agreement Form in blue ink
4. Obtain 2 letters of recommendation/reference
5. Mail the signed agreement form and 2 letters of recommendation to the address listed on the agreement form by the deadline
6. Receive notification shortly after the Board Meeting regarding application status

• Type of grant applying for (Crisis Intervention, Youth Development, or Educational Assistance)
• Application title
• Amount requested ($)
• Organization name and website
• Federal ID (501(c)3)
• Position held
• Full name
• Email address
• Phone number
• Street address
• City, state, zip

#### Required Narrative Questions - All Applicants

1. Briefly describe the Church or Agency applying for this grant, explaining your mission and those you serve
2. Share the problem your grant seeks to address and the solution you envision with Life Line Homes' funds
3. Summarize the budget for this project, listing specific dollar amounts, including any other groups or contributing agencies, and specifying the use of funds you seek from Life Line Homes

#### Required Narrative Questions - Crisis Intervention or Youth Development Grants

1. Share how you will evaluate whether your project has been successful and how you will report to Life Line Homes on use of funds
2. Share how you will continue to fund this project once the startup funds from Life Line Homes are exhausted

#### Required Narrative Questions - Educational Assistance Grants

1. Share briefly the background and need of each student for whom you seek assistance
2. What other assistance will the student(s) receive?
3. Share how you will report back to Life Line Homes the student(s)' progress and benefit from the funds provided

#### Submission Requirements

• Complete online application form
• reCaptcha verification
• Option to "Save As Draft" to return later
• Click "Submit For Review" when ready (directs to agreement form page)
• Download/print Agreement Form
• Sign in blue ink
• Mail with 2 letters of reference

#### Supporting Documents Required

• Agreement Form (signed in blue ink, mailed)
• 2 letters of recommendation/reference (mailed with agreement form)

#### Important Notes

• No grant will be considered unless a signed copy of the Agreement Form is postmarked by the deadline (January 31st or July 15th)
• Two letters of reference must be included with the agreement letter
• All areas of the online form must be completed for successful submission
