Home Matters to Arizona Portfolio of Funds

The Home Matters to Arizona Portfolio of Funds is a centralized application portal that provides applicants access to multiple funding opportunities focused on housing and social determinants of health. Projects submitted through this portal are evaluated by partner funders based on components of social determinants of health.

Vision: Capture funder interest and investment to facilitate quicker and more leveraged activation of housing and homelessness funding that aligns with the social determinants of health

What the Portfolio Does:

• Provides funder-specific investment opportunities
• Facilitates matching between developers and funders
• Offers one portal and one application for multiple funding opportunities
• Aligns projects with the Home Matters way: strong alignment to SDOH (Social Determinants of Health)

Who It's For:

• Organizations looking to secure funds for housing-related development projects
• 501(c)(3) organizations (though some funders may consider for-profit organizations or cities and towns)

Eligible Use of Funds: Applicants may request funding for:

• Housing development
• Associated physical components of social determinants of health
• Program components including:
  • Education
  • Social inclusion
  • Physical activity
  • Nutrition and healthy food
  • Healthcare access
  • Social services and behavioral health
  • Open/green space
  • Community leadership
  • Family and child support

Important Note: The "Housing is Healthcare Fund" (formerly "Home Matters Arizona Fund") is NOT a participating funder in this Portfolio. Applicants interested in that fund should contact Frieda Pollack at fpollack@lisc.org
