Overview: The Home Matters to Arizona Portfolio of Funds is a centralized application portal that provides applicants access to multiple funding opportunities focused on housing and social determinants of health. Projects submitted through this portal are evaluated by partner funders based on components of social determinants of health.

Vision: Capture funder interest and investment to facilitate quicker and more leveraged activation of housing and homelessness funding that aligns with the social determinants of health

Offers one portal and one application for multiple funding opportunities

Aligns projects with the Home Matters way: strong alignment to SDOH (Social Determinants of Health)

Associated physical components of social determinants of health

Program components including:
