Overview
A formula-based grant program administered by SAMHSA, designed to be an outcome driven grant program to support service delivery to individuals with Serious Mental Illness (SMI) and/or co-occurring substance use disorders (COD), who are experiencing homelessness or at imminent risk of homelessness. PATH funds street outreach, case management, and services not supported by mainstream mental health programs.
