Executive Summary:

3. Biographies: List and brief bios of key management staff and individuals leading the project (required)
4. Other commitments to the project (or to the organization if requesting general support); other pending grant applications (required)

Project Description:
5. Description of the project, its relevance to the mission of the agency and its alignment with the DVREDF mission (required)
6. Measurable impact of the project; how it advances the goals of the organization and DVREDF's goals of measurable economic development and/or job impact (required)
7. Who and/or which communities will benefit from the project (required)
8. How will the impact of the project be measured (required)
9. Activities and timeline for the project (required)

Grant Request:
10. Amount of the full project cost (required)
11. Amount requested from DVREDF (required)

#### Required File Uploads (Max 5 MB per file)

Allowed file types: .doc, .docx, .pdf, .jpg, .png, .xlsx, .xls

1. A list of the current Board of Directors and Officers complete with their affiliations
2. Data related to the project including a business plan, market analysis, feasibility study, etc.
3. Project budget including income and expenses (for general support requests, attach current organizational budget)
4. Three years of audited financial statements (if not required to produce audited statements agencies should submit their three most recent IRS 990s); current year P&L; projected organizational budget during the project and in the full year after the completion of the project
5. Additional attachments can be added as needed
