Grace Barnes Memorial Scholarship

Eligibility:

Students pursuing careers in allied health/nursing/medicine

Must demonstrate financial need

Must have been accepted to a recognized institution

Applicants' financial need and desire to pursue a career in allied health/nursing/medicine must be established

Must maintain full-time student status (minimum 12 quarter hours or 18 semester hours of course work)

Must maintain a minimum cumulative grade point average of 3.0

Award Details:

$750.00 per semester or $500.00 per quarter

Funds sent directly to the education institution on behalf of recipient

Used for: tuition, books, meal plan, and dormitory cost (does not cover off-campus housing)

Any unused money refunded to the Auxiliary

Requirements During Scholarship Period:

Recipient must maintain full-time student status

Must maintain minimum 3.0 GPA

Must provide written proof (official transcripts only) of scholastic standing each quarter/semester

Must provide tuition invoice for each quarter/semester

If student ceases course of study in related health field, scholarship funding will no longer apply

Failure to provide official transcript after each quarter/semester may result in termination without notice

Failure to provide tuition invoice as soon as registered may result in termination without notice

How to Apply

Application Process

Deadline: Completed application packets must be received no later than July 15

Required Documentation:
1. Completed application form (submitted electronically via form on website)
2. Three letters of recommendation:
- Must be signed by the author
- Must be emailed directly from author to volunteer@navicenthealth.org
- Subject line should read: "Grace Barnes Scholarship - Recommendation - Your Name"
- CANNOT be from relatives
3. Official transcript from most recent college or high school:
- Must be in sealed envelope from the institution providing it
- Mail to:

4. Proof of acceptance from recognized institution where scholarship will be used

Application Form Sections:

Section I: Demographics (name, address, contact info, date of birth, parents' information)

Section II: Academics (high schools/colleges attended, courses pursued, entrance exam status, honors, school selection, course of study, ultimate goals)

Section III: Financial (plans for meeting expenses, employment status)

Section IV: References (names and addresses of three adults not relatives)

Section V: Goals (reasons for career selection, work experience, volunteer work, attitude/interest statements, scholarship application reasons)

Student Certification:

Applicant must certify that information is true, correct, and complete

Must acknowledge reading policies and procedures

Must understand the Scholarship Committee's decision is final

Must agree to provide additional personal/financial information if requested

Must understand funding is for tuition, books, meal plan, and dormitory costs only

Must understand scholarship terminates if student ceases course of study in health field
