Application Process

1. Online Application Form - Complete the GOAP Scholarship Application at ysausa.org/programs/scholarships/goap-scholarship-application/

2. Required Information:
- Student information (name, DOB, grade, school)
- Parent/guardian contact information and emergency contact
- Medical information (allergies, conditions)
- Free and Reduced Lunch Program participation status
- Previous YSA scholarship history

3. Essay Questions (choose appropriate set for your situation):
- Why should you be considered for a scholarship?
- What are your goals for the program?
- How does being part of this program affect your life?
- How does your activity help you gain YSA core values (Sportsmanship, Perseverance, Optimism, Respect, Teamwork) and what do they mean to you and your future?

4. Equipment List - If requiring equipment, list items needed

5. Terms & Conditions Agreement:
- Agree to pay commitment fee in full
- Child will fully participate in program
- Understand that irregular attendance results in ineligibility for following year
- Will arrange timely pickup
- Accept liability release and medical care authorization
- Must sign acknowledging reading and understanding terms
