Get Out & Play and ACTiV8 Partial Scholarship

Program Focus: Combined scholarship opportunity for students seeking participation in either or both Get Out & Play (elementary winter sports) and ACTiV8 (middle/high school athletic programs) programs.

Eligibility:

  • Students in grades 1-9

  • Households with annual income under $120,000

  • Automatic eligibility: Students qualifying for Free and Reduced Lunch Program (no application needed)

  • Income verification may be required (paystubs, tax forms, W-2s)

  • Can apply for multiple children on single application (up to 3 students)
  • Award Coverage:

  • Partial tuition assistance

  • Supports participation in GOAP and/or ACTiV8 programs

  • Equipment assistance when needed

  • Commitment fee support
  • Geographic Scope: Summit and Wasatch Counties, Utah; serves students from multiple school districts

    Requirements:

  • Full participation in all program sessions

  • Must arrive on time and stay for duration of each program day

  • Regular attendance required

  • Payment of commitment fee in full

  • Two thank you notes to YSA sponsors (due by final day of program)

  • Frequent tardiness/absence may result in withdrawal and ineligibility for future scholarships

    How to Apply

    Application Process

    1. Online Application Form - Complete the GOAP & ACTiV8 Partial Scholarship Application at ysausa.org/get-out-play-and-activ8-partial-scholarship-application/

    2. Student Information (up to 3 students):
    - Name, birth date, grade level
    - School selection from comprehensive school list
    - Can apply for multiple children on single form
    - Includes schools from Park City, North Summit, South Summit, Wasatch, Salt Lake City areas

    3. Parent/Guardian Information:
    - Full name, relationship to student
    - Complete address with state and ZIP
    - Email and phone number (required)
    - Annual household income range (required)
    - Total household members under age 18
    - Total household members age 18+

    4. Financial Information:
    - Annual household income (select from ranges $50,000-$120,000)
    - Income above $120,000 does not qualify
    - Explanation of need for partial scholarship with extenuating circumstances
    - Detailed description of financial situation (may request additional information)

    5. Program Information:
    - Previous YSA scholarship history (yes/no)
    - Which programs applying for (GOAP, ACTiV8, or both)
    - Detailed explanation: "What would your child be doing without this scholarship?"

    6. Scholarship Agreement:
    - Agree to pay commitment fee in full
    - Child will fully participate in all program sessions
    - Understand that irregular attendance results in ineligibility for following year

    7. Terms & Conditions:
    - Child must arrive on time and stay for duration of each program day
    - Frequent tardiness or absences may result in program withdrawal
    - Attendance records will be considered for future scholarship applications
    - Child must write two (2) thank you notes to YSA sponsors
    - Thank you notes must be submitted by final day of program session
    - Must check box confirming read and agreement to all terms

    8. Submission Options:
