ACTiV8 Partial Scholarship

Application Process

1. Online Application Form - Complete the ACTiV8 Scholarship Application at ysausa.org/programs/scholarships/activ8-scholarship-application/

2. Student Information (up to 3 students per application):
- Name, birth date, grade level
- School selection from provided list
- Can apply for multiple children on one form

3. Parent/Guardian Information:
- Full name, relationship to student
- Complete address
- Email and phone number
- Annual household income (select range: $50,000-$120,000)
- Total household members under age 18
- Total household members age 18+

4. Financial Information:
- Annual household income range (required)
- Explanation of why seeking partial scholarship or extenuating circumstances
- Detailed description of financial need (may request additional information)

5. Program Information:
- What programs applying for
- Previous YSA scholarship history (yes/no)
- What child would be doing without scholarship

6. Scholarship Agreement:
- Agree to pay commitment fee in full
- Child will fully participate in all program sessions
- Understand irregular attendance results in ineligibility for following year

7. Terms & Conditions:
- Must arrive on time and stay for duration
- Frequent tardiness/absence may result in withdrawal
- Attendance records considered for future applications
- Required to write two thank you notes to sponsors
- Must check box indicating read and agreement to terms

8. Submission: Use "Save and Continue Later" button to complete form over multiple sessions or submit completed form
