Return signed grant acceptance prior to fund release

Adhere to grant terms throughout grant period

Ensure all publicity mentions Foundation support

Submit fiscal summary and project evaluation within 60 days of project completion (signed by CEO/project leader)

Submit six-month evaluation and fiscal summary if project exceeds six months

Submit photographs of project (where appropriate) for Foundation use on website, newsletter, and brochures

Failure to submit reports may jeopardize future funding
