  • Signed cover letter from authorized executive (Board President or CEO) including 501(c)(3) number

  • Agency narrative including: history, description of services, constituents served

  • Explanation of how proposal fits agency mission

  • Description of organizational capacity (staff qualifications, best practices)

  • Community impact and outcome measurements

  • Sustainability plan

  • Details of collaboration (letters of support, memorandums of understanding)

  • For continuing programs: plan for securing future funding

  • All pertinent signatures on application form
  • Post-Award Requirements:

  • Return signed grant acceptance prior to fund release

  • Adhere to grant terms throughout grant period

  • Ensure all publicity mentions Foundation support

  • Submit fiscal summary and project evaluation within 60 days of project completion (signed by CEO/project leader)

  • Submit six-month evaluation and fiscal summary if project exceeds six months

  • Submit photographs of project (where appropriate) for Foundation use on website, newsletter, and brochures

  • Failure to submit reports may jeopardize future funding
