Froedtert Holy Family Memorial Hospital Community Grant Program

The Froedtert Holy Family Memorial Hospital Community Grant Program aims to support organizations enhancing community health through collaborative practices in Manitowoc County. Grants up to $25,000 are available for non-profits addressing priority health needs and social determinants, focusing on underserved populations. The program encourages initiatives that improve health status, promote education, employment, safety, and foster collaboration among organizations while aligning with community health improvement priorities.

Froedtert Holy Family Memorial Hospital Community Grant Program
It is the goal of the grant program is to help support organizations that are advancing community health through collaborative and evidence-based practices targeting the priority health needs in Manitowoc County. Qualifying organizations can apply for funding support up to $25,000.
Purpose
The Community Grant Program’s highest funding priority is given to programs or projects that address the hospital’s priority health needs, social drivers of health and/or target underserved populations.Funding is available for non-profit organizations in good standing that represent a range of demographics within Manitowoc County.
Qualifications
IRS tax-exempt organization(s) classified as a 501(c) that provides services in the Froedtert Holy Family Memorial Hospital service area (Manitowoc County).
Projects that:
Contribute to improving the overall health status of the community, including social determinants of health:Education (e.g., Literacy programs, higher education programs, health education programs)Employment (e.g., Soft skills training, apprenticeship programs, internship programs)Health (e.g., Access to quality care, healthy food, screening, behavior health)Safety (e.g., Transportation, housing, green space access, violence education and prevention)Establish new programs or support existing initiatives that improve health and wellnessSupport communities in Manitowoc County, specifically marginalized communitiesFoster collaboration between organizationsIncorporate population health and align with Froedtert Holy Family Memorial Hospital Community Health Improvement Plan priorities
