Funder: Holy Family Memorial (HFM)

Purpose: To assist patients on their wellness journey by helping them participate in various wellness programs, regardless of ability to pay. The fund was created through the generosity of donors who believe in helping others achieve healthier lives.

Supported Programs

The Wellness Fund provides financial assistance for:

6-month Wellness Center membership

Parkinson's Disease Class – Exercise class for patients in all stages of Parkinson's disease to maintain function and improve balance

Pain Neuroscience Education (PNE) Class – Helps you take control of chronic pain and health to regain quality of life

Health and Fitness Coaching

Eligibility and Requirements

• Ownership Investment: Applicants must make a nominal ownership investment
• Participation Commitment: Must commit to participate in selected wellness program
• Application Required: Complete and submit application
• Processing Time: Allow 10 business days for application review

Award and Notification Process

• Once approved and qualified for financial assistance, applicants receive a letter detailing:
- The amount of financial assistance awarded - Instructions for enrolling in wellness programs
