Employee and Dependent Scholarships

Funder: Trilogy Health Services Foundation

Purpose: To provide educational support to employees and their dependents pursuing higher education.

Full-time and part-time employees of Trilogy Health Services and Synchrony Health Services

Must have completed at least 6 months of service

Must demonstrate financial need

Must be in good standing with the company

Must meet charitable class income guidelines (household income at or below 350% of federal poverty guidelines)

Geographic Scope: Employees of Trilogy Health Services and Synchrony Health Services locations

What They Fund: Scholarships for employee education and dependent college education

What They Don't Fund: Past or future semesters; scholarships are only valid for 180 days and must be applied to the upcoming semester

- Dependents: Complete the Dependent Scholarship Form
3. Submit form prior to application deadline
4. Foundation Scholarship Committee reviews applications
5. Receive notification of decision
6. Apply scholarship to student account at educational institution (employee/dependent responsibility)

Scholarship checks valid for 180 days only

Must be applied to upcoming semester only

Cannot be applied to past or future semesters
