Community Health Improvement Fund Grants: Treasure Valley and Multi-Region 2025 Application RequirementsApplications should address the social determinants (or drivers) of health (SDOH), or our top-ranking community health needs as identified by our 2023 Community Health Needs Assessment (CHNA) covering 2024, 2025 and 2026: Boise, Meridian, Elmore, McCall and Nampa Safe, affordable housing and homelessnessBehavioral health, including mental health and well-being and substance misuseAccess to affordable health care, including oral and vision Wood River Access to health-related services (including language and cultural barriers)Mental well-being (including suicide and substance misuse)High cost of living (including housing, caregiving, and early learning)

Eligible applicants will operate as a non-profit including:Have current 501(c)(3), tax-exempt status under IRS Code Section 170 or be a government agency/department or public education institution.Operate under written articles of incorporation and bylaws or other written documents or statutes that define the applicant’s purpose, membership, management, and operation.Operate on a non-discriminatory basis in employment, recruitment of volunteers and delivery of services.Demonstrate effective project or program performance, financial responsibility and accountability.
