Bobby Jones Open (BJO) Equipment Grant

Purpose: Assist qualifying ASAP members with purchase of adaptive equipment and assistive devices to improve quality of life and promote independence.

Award Details:

• Grant amount: Up to $1,000 per award
• Payment method: Checks made directly to vendor (not to individual)
• Focus: Adaptive equipment and assistive devices

Eligible Equipment Examples:

• Ramps
• Therapeutic bicycles
• Accessible showers
• Lift chairs
• Wheelchair accessories
• Other adaptive/assistive devices

Eligibility Requirements:

• Must be ASAP member for at least 6 months prior to applying
• Must have diagnosed syringomyelia or Chiari malformation
• Must provide written quote for total cost of item(s)
• Checks made directly to vendor

Geographic Scope: United States

Background: The Bobby Jones Open golf tournament has enabled numerous ASAP members to improve quality of life and enjoy more independent lifestyles.

Application Process

1. Download BJO Equipment Application Form - Available on ASAP website

2. Required Documentation:
- Completed BJO Equipment Application form
- Written quote(s) for total cost of adaptive equipment/assistive devices
- Evidence of ASAP membership for minimum 6 months
- Documentation of SM/CM diagnosis

3. Submission:
- Mail completed application and supporting materials to ASAP
- Include vendor quote(s) with application

4. Award Process:
- Upon approval, ASAP issues check directly to vendor
- Payment not made to applicant
