Grantee to Host the Disability Hub RFP (MN) Objective The Minnesota Department of Human Services, through its Disability Services Division (STATE), is seeking Proposals from qualified Responders to deliver Disability Hub (Hub) services established by the commissioner under Minnesota Statute Section 256.01, subdivision 24. The term of any resulting contract is anticipated to be for 1.5 years. Current Model and Proposed Transition Since 2007, the Hub has operated under a two-provider model to deliver statewide services. To enhance efficiency, consistency, and overall service quality, STATE is transitioning to a single-provider model for statewide service delivery. This transition provides an opportunity to streamline operations while maintaining the Hub’s commitment to high-quality, accessible support. A single-provider model will allow for more coordinated service delivery and ensure that all Minnesotans with disabilities continue to receive the guidance and resources they need. To be considered under this RFP, Responders must agree to: Have a commitment and physical presence in Minnesota. Responder must understand the needs of the local community.Provide Disability Hub MN services in alignment with the Standards and Assurances, which define the essential components, values, and quality expectations for service delivery. These standards guide how the Hub operates to ensure it remains a trusted, person-centered, and unbiased resource for people with disabilities across Minnesota. As part of their submission, Responders must attest that they meet the above criteria. Funding Grant funding, through a state legislative appropriation, is the main and ongoing source of funding for the contact center service. The STATE will select one (1) responder. Year 1 (this year) – up to $550,000 for transition planning and hiring Year 2 (next year) – up to $3,135,633 for full implementation STATE may extend the contract up to a total of five (5) years. If the contract is extended, STATE anticipates awarding up to $3,135,633 per year to the selected Responder.Eligibility
