Application Process

Applicants must complete the online Scholarship Application Form which includes the following sections:

1. Personal Information
- First Name, Last Name, Middle Initial
- Phone, Email
- Street Address, City, State, Zip Code

2. Awards, Honors, and Community Service
- Brief description of awards, honors, and community service

3. College Education
- School Name and Address
- Attendance dates (From/To)
- Graduation status (Yes/No)
- Degree type
- MLS Program (if applicable)

4. High School Education
- School Name and Address
- Attendance dates (From/To)
- Graduation status (Yes/No)
- Degree type

5. Interest & Documentation
- Essay: Why you are pursuing a career in Immunohematology (blood banking)
- Essay: Describe your interest in working with Vitalant Reference Laboratory
- Essay: Describe any relevant experience (academic, volunteer, or work)
- Upload Resume or CV

6. Disclaimer and Signature
- Certification that answers are true and complete
- Type name as signature
- Submit application
