Vision Start Program

Geographic Scope: North Dakota (serves multiple locations including Beulah, Bismarck, Carrington, Devils Lake, Dickinson, Fargo, Grand Forks, Jamestown, Kenmare, Linton, Mandan, Minot, Stanley, and Williston)

Purpose: Provides free eye exams and eyeglasses to eligible children ages 3-7 in North Dakota.

Eligibility:

• Child must be age 3-7 years old
• Family income must be at or below the federal poverty guidelines based on household size:
  • 2 people: $54,930
  • 3 people: $69,090
  • 4 people: $83,250
  • 5 people: $97,410
  • 6 people: $111,570
  • 7 people: $125,730
  • 8 people: $139,890
  • For families with more than 8 persons, add $4,720 per additional person

Vision Start Application Process

Eligibility Screening:

4. Provider Selection
   • Select city from available locations
   • Select practice
   • Select eye care provider
   • Contact the selected clinic directly to schedule the appointment and reference the Vision Start Program
5. Submission
   • Click Submit button
   • Information will be emailed to: Optometric Foundation of North Dakota, the selected eye clinic, and applicant
   • Applicant must contact the provider to schedule the appointment
