Vision Start Program

Funder: Optometric Foundation of North Dakota

Geographic Scope: North Dakota (serves multiple locations including Beulah, Bismarck, Carrington, Devils Lake, Dickinson, Fargo, Grand Forks, Jamestown, Kenmare, Linton, Mandan, Minot, Stanley, and Williston)

Purpose: Provides free eye exams and eyeglasses to eligible children ages 3-7 in North Dakota.

Eligibility:

  • Child must be age 3-7 years old

  • Family income must be at or below the federal poverty guidelines based on household size:

  • - 2 people: $54,930
    - 3 people: $69,090
    - 4 people: $83,250
    - 5 people: $97,410
    - 6 people: $111,570
    - 7 people: $125,730
    - 8 people: $139,890
    - For families with more than 8 persons, add $4,720 per additional person

    4. Provider Selection
    - Select city from available locations
    - Select practice
    - Select eye care provider
    - Contact the selected clinic directly to schedule the appointment and reference the Vision Start Program

    5. Submission
    - Click Submit button
    - Information will be emailed to: Optometric Foundation of North Dakota, the selected eye clinic, and applicant
    - Applicant must contact the provider to schedule the appointment
