Vibrant Communities: Health Grant Opportunity

The Vibrant Communities: Health Grant Opportunity supports nonprofits in Jefferson County by funding initiatives that promote holistic wellbeing. This includes enhancing mental, social, spiritual, and physical health through operational support for organizations focused on Medicaid enrollment, public education campaigns, and community-building efforts. Eligible organizations must be 501(c)(3) nonprofits serving Jeffco residents, demonstrating measurable impact in the community. This grant aims to foster vibrant, connected communities in Jeffco.

We provide grants to nonprofits working to ensure all of Jefferson County (Jeffco) is thriving. Our funding opportunities are developed through active community listening to support collective wellbeing, economic freedom and vibrant communities. These grants provide resources for nonprofits to advance their mission and make good happen in Jeffco.

Eligibility

This grant opportunity provides general operating support to community partners who are advancing work in one or more of the following:Support organizations that are increasing Medicaid and Medicare enrollment, navigation and utilization.Support local, regional and statewide advocacy and public policy to reduce the administrative burden of Medicaid and Medicare, for both providers and patients.Support public education campaigns around enrollment, the value of preventative care and the value of oral health.Support organizations that provide low or no-cost mental, physical and dental health services to community members.Support public education campaigns around the value of social connection and consistent participation in groups and clubs.Support social and faith organizations that aim to build social connections through ongoing community service with consistent participation.Support programs to bridge community networks fostering connections between different identity groups such as age, race, lived experience like intergenerational mentoring programs.Support storytelling efforts to highlight the benefits of social connection and lower the barriers to participate in social activities.Your organization must also meet the following requirements to be eligible for this grant:Organization Eligibility: Your organization must be a 501(c)(3) nonprofit, or an organization operating under fiscal sponsorship of a 501(c)(3) nonprofit. If your organization is another classification but you believe your organization deserves consideration, please contact us directly.Geographic Eligibility: Colorado Gives Foundation is focused on helping Jefferson County thrive. As such we only fund organizations who are committed to serving Jefferson County residents and can demonstrate their impact in Jeffco with number of Jeffco residents served, percentage of budget applied to Jeffco or concrete programs that are specific to or focused on Jeffco populations.
