UPHA Foundation Grant (General Grants)

      Special Considerations

    • No single grantee shall receive all available Foundation funding in one calendar year
    • Foundation may give special consideration to programs offering matching grant possibilities
    • Grantee must demonstrate financial responsibility for duration of grant allocation
    • Grantee must provide expenditure report within one year of receiving grant (failure to report precludes future awards)
    • If grant purpose modified by grantee, Foundation reserves right to terminate agreement and funding
