Travel Support Package

Geographic Scope: United States only

Purpose: Help individuals with SMA travel by air more safely, comfortably, and with greater peace of mind

Eligibility:

• Age: 13 years of age or older (expanded in March 2026 to include teens; previously available only to adults)
• Diagnosis: Confirmed SMA diagnosis required
• Residency: Must reside within the United States
• Applicants: Parents or guardians may assist teens with the application process

What is Included:
The Travel Support Package provides recommended assistive travel equipment:

• Portable transfer sling
• AirTag and holder for tracking mobility devices
• Supportive travel pillow
• Customizable signage materials
• Portable inflight foot stool

How to Apply

1. Visit the Request Form: Access the Travel Support Package request form on Cure SMA's website
2. Complete the Application: Fill out the form with required information
3. Provide Required Information:
   • Confirmed SMA diagnosis
   • Proof of U.S. residency
   • Age confirmation (13 years or older)
4. Submit: Submit completed form through the online portal

Assistance Available: Parents or guardians may assist teens with the application process as needed.
