Across therapeutic areas, we support disease awareness efforts, patient empowerment and advocacy, and linkages to care. Our preference is in support of programs focused on health system improvement with measurable outcomes over fundraising activities. Cardiovascular & Metabolism Disease Areas Stroke Prevention in Atrial Fibrillation, in particular populations who are obese.Thromboembolic Disease, patients at risk for VTE, both adult and pediatric populations.Vascular Protection (CAD/PAD) and prevention of amputation Areas of Interest Overall, we will support programs and initiatives that assist patients and caregivers impacted by the diseases noted below in the Cardiovascular therapeutic area, with a preference toward: Education of patients and caregivers on the respective disease areasFamily/caregiver support, wellness education, advocacy supportInnovative efforts to empower patients to manage their diseaseTools to assist patients & families related to anti-coagulant therapyEfforts to build skills of professionals and caregivers to improve patient outcomesEfforts to address healthcare disparities for marginalized or at-risk groups (outreach & awareness, education to policy makers, stigma & cultural competence initiatives)Advocacy initiatives on access to care to treat individual patient needs
