Cardiovascular Disease Prevention Program The New Jersey Department of Health (NJDOH) - Cardiovascular Disease Prevention Program (CVD) seeks to fund evidence-based strategies to prevent and manage cardiovascular disease in adult, high-burden populations/communities in Newark, contributing to improved health outcomes (reduction of hypertension rates in census tracts of focus), through population-based, evidence-based system interventions.
