The CAPABLE Model: An Aging in Place Intervention

Subtitle: An Aging in Place Intervention for Older Adults in Central Texas

Geographic Scope: Central Texas (Bastrop, Caldwell, Hays, Travis, Williamson counties)

St. David's Foundation funds replication of the Community Aging in Place—Advancing Better Living for Elders (CAPABLE) model, developed by the Johns Hopkins School of Nursing. CAPABLE uses a multidisciplinary team approach to help vulnerable seniors live safely in their homes.

Model Description: The CAPABLE team consists of three professions:

• Occupational therapist (OT)
• Registered nurse (RN)
• Handyperson

The program is guided by client-directed care—seniors determine their own goals for functional improvement and define the meaningful daily activities they want to achieve.

Target Population:

• Income: Low-income (generally under 250% FPL)
• Age: Over 60 years old
• Location: Central Texas
• Criteria: Desire and cognitive ability to remain in home but have difficulty with activities of daily living that threaten their ability to remain safely at home

Eligibility: Tax-exempt 501(c)(3) non-profit organizations and/or public entities

Program Focus: Complements the Foundation's existing commitment to support aging in place for vulnerable seniors
