The CAPABLE Model: An Aging in Place Intervention
Subtitle: An Aging in Place Intervention for Older Adults in Central Texas
Geographic Scope: Central Texas (Bastrop, Caldwell, Hays, Travis, Williamson counties)
Overview: St. David's Foundation funds replication of the Community Aging in Place—Advancing Better Living for Elders (CAPABLE) model, developed by the Johns Hopkins School of Nursing. CAPABLE uses a multidisciplinary team approach to help vulnerable seniors live safely in their homes.
Model Description: The CAPABLE team consists of three professions:
The program is guided by client-directed care—seniors determine their own goals for functional improvement and define the meaningful daily activities they want to achieve.
Target Population:
Eligibility: Tax-exempt 501(c)(3) non-profit organizations and/or public entities
Program Focus: Complements the Foundation's existing commitment to support aging in place for vulnerable seniors
