SWACH Collaborative Funding Model

Funder: Southwest Washington Accountable Community of Health (SWACH)

Status: Ongoing funding program

Award Amount: Over $1.1 million awarded to six organizations in 2025

Geographic Scope: Southwest Washington (Clark, Klickitat, Skamania counties and Columbia Gorge region)

Background: SWACH's Strengthening our Collective Futures initiative supports programs led by and for the communities they serve. The funding aims to advance community health, equity, and well-being through community-driven solutions.

Focus Areas

Projects addressing local needs including:

* Culturally grounded wellness
* Food access and social drivers of health
* Maternal and perinatal mental health
* Substance use recovery and harm reduction
* Reentry support for formerly incarcerated individuals
* Health-related social needs (housing, transportation, utilities, food)
* Community health worker capacity building

Eligibility

* Community-based organizations
* Organizations led by and reflecting the communities they serve
* Programs with lived experience and local leadership

Support Beyond Funding

In addition to grant funding, SWACH provides grantees with:

* Capacity-building resources
* Technical assistance
* Training and evaluation support
* Mentorship

* Pacific Islander Community Association of Washington (PICA-WA) - Wellness Navigation program in Clark County
* Gorge Perinatal Mental Health Initiative - Peer-led support groups for new parents in Columbia Gorge
* One Community Health, Oregon - Health-Related Social Needs screening and navigation in Klickitat and Skamania counties
* The Sober Place dba Couve Collective - Recovery café expansion in Vancouver
* Nch'i Wana Housing - Outreach and support for Native communities in Klickitat and Skamania counties
* The Foundation WA - The Restored Transitional Complex for reentry and healing
