Senior Oral Health Partnership Program

Geographic Scope: 15-state enterprise and Washington, D.C. (AL, CA, DE, DC, FL, GA, LA, MD, MS, MT, NV, NY, PA, TX, UT, WV)

Launch Date: 2022

Multi-year program launched in 2022 to identify and address systemic causes of poor oral health among older adults. The program was expanded in May 2025 with an additional $5 million commitment to the Mississippi Delta region over five years.

Focus Areas:

• Senior oral health crisis among older adults in the U.S.
• Older adults living in poverty (1 in 4 seniors in poverty have lost all natural teeth)
• People of color
• Older adults with mobility issues preventing access to care
• Community-based partnerships addressing systemic causes of poor oral health

Background:

• Driven by 2021 National Institutes of Health Oral Health in America report
• Recognized as a public health crisis
• Part of broader 2022 broadened philanthropic focus
• Since 2012, Community Care Foundation has granted more than $170 million total
