    2. Requested amount from Rotary Club

    5. Number of individuals impacted by the funded program (excluding employees or volunteers)
    6. Detailed description of the community problem or need addressed by the program
    7. Detailed breakdown of spending intent:
    - Specify each need
    - Indicate if funding source is Rotary Club or alternative sources (other grants, corporate support, etc.)
    8. Certification statement signed by authorized representative
    9. Authorized signatory name, title, and phone number
    10. Optional: Name of Rotarian recommending the application
