Health Care Workforce
This RFP will support integrated primary care teams that help achieve
healthy outcomes in collaboration with people from underserved
communities. We will prioritize existing provider teams working to
create a positive patient experience, implementing effective care
management, and focused on both individual and population health.

Cambia Health Foundation is the corporate foundation of Cambia Health Solutions, which is dedicated to

RFP Strategy
Primary care prevents serious health issues, improves chronic disease management and increases access
to behavioral health services. Integrated primary care teams provide high quality care with a whole-person
approach addressing physical health, mental health and social needs together.
Integrated primary care teams are dedicated clinical and non-clinical providers working together to
improve health outcomes by addressing the full spectrum of patient needs through coordinated, efficient
and patient-centered approaches. These teams work in organizations such as community health centers,
rural health clinics, certified community behavioral health clinics and their existing strategic partner
organizations. The aim of this RFP is to support integrated primary care teams providing whole-person
health services to underserved communities – including populations experiencing higher poverty rates,
insurance gaps, complex health needs, and barriers to health and social services.
This RFP will support integrated primary care teams helping to achieve healthy outcomes in collaboration
with people from underserved communities. We believe there are many different approaches an
organization or partnership may take to achieve these goals. We are particularly interested in supporting
primary care teams working to optimize collaboration with people being served, providing effective care
management, and focusing on both individual and population health.
Support for existing primary care teams and collaborations will be prioritized through one or more of the
following strategies:
• Training and career development to advance integrated primary care
• Programs addressing stressful working conditions and burnout to enhance provider wellbeing
• Supervised, paid internship and practicum experiences for students
• Support for technologies that enable care teams to provide quality in-person or remote care through
various access points
• Designated funds addressing social barriers identified during patient care
Outcomes and impacts
Strong applications will clearly define the population served, demonstrate care team buy-in, and connect
directly to at least one of these outcomes:
• Increase provider training or career advancement in team-based models of care focused on
increasing access, patient experience and improved outcomes.
• Expand behavioral health and social service supports.
• Increase provider well-being and job satisfaction.
• Advance clinical and non-clinical primary care team members working together to understand and
reflect the communities served.
We recognize multiple strategies exist to support primary care teams in providing effective, equitable
care. Applications should:
• Identify the most pressing needs where actions will have the greatest impact, leverage organizational
strengths, and create opportunities for meaningful, sustainable change.
• Be focused on a specific priority rather than trying to address every possible component of primary
care support.
• Include how connections to behavioral health services are made within the primary care framework.

Eligibility
• Applicants should be a single organization providing integrated primary care services (i.e., access to
physical, behavioral and social services on-site, virtually or in home/community settings) or an
organization on behalf of an integrated partnership between a primary care center(s) and social
service provider(s).
• Preference for existing programs or collaborations, with new programs or collaborations considered
with viable pathways to sustainability.
• Proposed work must be focused on serving populations within Oregon, Washington, Idaho, Utah
and/or sovereign American Indian tribes within those states.
• Applicant organizations must be an active U.S. 501(c)(3) organization; accredited school or university;
government or public agency; federally recognized Indian Tribal entity. Fiscal agents are eligible to
serve as applicants on behalf of eligible organizations doing charitable work.
• Organizations/entities that do not offer all components of integrated care either directly or through
established collaborations will not be eligible to apply. If you are not sure your proposed work or
organization is eligible to apply, we encourage you to reach out to our team and ask.

Funding guidelines
We ask for requested grant amounts between $30,000 and $60,000 and one application from an
organization or collaboration. We anticipate awarding 8-12 grants throughout our geographic region in this
RFP. We estimate grant start dates to be in October 2025. The grant period can last up to 24 months after
the initial start date. Please note that these are general guidelines for planning purposes. Details will be
