Geographic Scope: Eastern Frontier Country, Oklahoma - specifically serving:

  • Youth Serving Organizations: Must describe staff and volunteer training, screening, and safety protocols if serving youth under age 18

    - Mission statement
    - Number of employees
    - Program name (specific program requesting funding, not organization name)
    - Amount of funding requested

    3. Needs Assessment
    - Describe specific local need(s) the program addresses
    - Provide data and sources demonstrating local need
    - Describe program including frequency, duration, cost to participants, and activities
    - Explain how program addresses identified need(s)

    4. Population Served
    - Key demographic characteristics (age, race/ethnicity, veteran status, disability status, etc.)
    - Percentage of individuals served who are low-to-moderate income
    - Brief description of how low-to-moderate income status is determined
    - Select all communities with program access during 2026-2027
    - Specify communities within Seminole and Hughes counties if applicable

    5. Service Data
    - Total individuals served in 2025 (unduplicated)
    - Number of individuals served in 2025 by ZIP code
    - Anticipated individuals to be served July 2026-June 2027 (unduplicated)

    6. Evaluation and Outcomes
    - Describe how program outcomes, progress, and effectiveness will be measured
    - Include tools or data used for measurement
    - Provide key evaluation results and measurable outcomes from 2025 with supporting data

    7. Funding Information
    - List all current and pending funding sources with amounts if known
    - If applicable, outline how 2025 United Way funding was utilized
