QI Grant Project Opportunity: Using a Shared Plan of Care for Children and Youth with Special Health Care Needs (CYSHCN)

Status: This was a past funding opportunity (application deadline was November 16, 2018)

Funder: Children's Health Alliance of Wisconsin's Medical Home Initiative, with funding from Wisconsin Department of Health Services, Division of Public Health, Bureau of Community Health Promotion, Family Health Section, Children and Youth with Special Health Care Needs Program

Geographic Scope: Wisconsin medical practices

Focus Areas:

Quality improvement projects using Shared Plan of Care approach for children and youth with special health care needs

Eligible conditions include: Attention-Deficit/Hyperactivity Disorder (ADHD), Juvenile Rheumatoid Arthritis (JRA), medically complex children, and those transitioning from pediatric to adult health care

What is a Shared Plan of Care:

A living document completed by parents and health care providers

Includes information necessary to assure issues affecting a child's health are identified and accessible across systems

Documents activities and accountability for addressing those activities

Eligibility:

Wisconsin medical practices serving children and youth with special health care needs

Pediatricians who can attest to meaningful participation in the project are eligible for 25 Maintenance of Certification (MOC) Part 4 credits through the American Board of Pediatrics upon project completion

Funding Tracks:

Based on available funds up to $100,000 total

Track 1: Up to $10,000

Track 2: Up to $20,000

Key Information:

Teams will use quality improvement approaches to accelerate their learning

Over 20 Wisconsin clinics participated in Shared Plan of Care pilot projects since 2016

Clinics report streamlined communication between primary providers, other health care providers, and mental health counselors

Families report improved understanding and communication about their child's needs
