Disabled and Displaced Student Scholarship Program

Program Name: Credit 341 - Corporate Contribution to School Tuition Organizations for Displaced Students or Students with Disabilities

Geographic Scope: Diocese of Phoenix, Arizona (38 Catholic schools) and St. Michael Indian School in St. Michael, Arizona (Diocese of Gallup, New Mexico)

Purpose: Provides tuition scholarships to students with disabilities and displaced students (including foster care) attending Catholic schools

Eligible Student Categories

• Students with a MET (Meets Expectations of Timeline)
• Students with an IEP (Individualized Education Plan)
• Students with a 504 Plan
• Students who have been in the Arizona foster care system
• Students with a parent in the military (displaced)
• Neuro-diverse learners and students with diversified learning needs

How Funding Works

• Corporate Funding Only: Corporations filing as S/C Corp, LLCs filing as S Corp, and insurance companies can direct state tax liability to this program
• Up to 100% of state tax liability can be directed
• This is a tax credit, not a donation
• June 30 deadline for corporate tax liability commitments
• Both large companies and small firms can participate in either Low Income program, Disabled/Displaced program, or both

How to Apply

1. Students/Families: Contact your Catholic school's financial aid office or principal to inquire about Disabled/Displaced Scholarship eligibility
2. Documentation Required:
   • Verification of qualifying condition (MET, IEP, 504 Plan, foster care status, military parent)
   • Enrollment at Diocese of Phoenix Catholic school
   • Financial information may be required
3. School Coordination: Schools coordinate with Catholic Education Arizona to process applications and distribute scholarships
4. For Corporate Donors:
   • Contact Catholic Education Arizona to set up corporate partnership
   • Complete Department of Revenue application process
   • Deadline: June 30 for tax liability commitments
   • Work with CEA professional staff (application process described as "easy")
