Vision We envision an equitable Pajaro Valley with health, wellness, and a full quality of life for all. Grants Through our grantmaking, the Community Health Trusts aims to build health equity and promote wellness for all in the Pajaro Valley. These goals are a reflection of our latest Strategic Plan, which focuses on driving significant impact in a changing health environment. The request must align with one or more of our strategic focus areas: Building Health Equity:An equal and fair opportunity for every individual to attain the highest level of health and wellness regardless of socioeconomic barriers, and/orIncreasing Wellness in Pajaro Valley: The active pursuit of activities, choices, and lifestyles that emphasize preventing illness and improving quality of life, as opposed to emphasizing disease treatment.Eligibility

The applicant organization must be:a nonprofit, 501 (c) 3 tax-exempt organization;a school-based health program; orhave a 501 (c) 3, tax-exempt organization as a fiscal sponsor.The request should serve individuals living in our primary service area.Consists of three zip codes: 95076, 95019, and 95004. While there is no formal policy about the number of consecutive years an organization is eligible to receive support from the Health Trust, grantees are typically invited to take a year’s hiatus from applying after four consecutive grants.Ineligibility
In general, the Health Trust's Board prefers not to fund programs or projects administered by government agencies, with the exception of school-based health programs closely aligned with the CHT's mission. We do not fund:Individuals, with the exception of the Health Trust’s scholarship programs.Religious organizations for purposes of evangelizing, proselytizing, or converting.Endowments, building campaigns, annual fund appeals, fundraising events or celebrations.Commercial ventures.Previously completed activities.Debt reduction.Focus Areas & Funding Uses
