Contra Costa Health Staff Grant Requests (CCRHF)

The Contra Costa Regional Health Foundation funds Contra Costa Health (CCH)-related projects that are innovative, mission-aligned, support the Vital Conditions for Wellbeing, and have no other means of support. Grants are intended for efforts that go beyond standard operations, where a clear need exists and a practical solution is ready to be implemented. Eligibility is restricted — a proposal must be approved by executive staff before submission and include division manager sign-off, so this is open to Contra Costa Health divisions and staff rather than to outside nonprofits. Strong applications define the problem, the population served, expected outcomes, a clear project lead, a realistic timeline, an implementation plan, other funding sources explored, a detailed budget, a sustainability plan and an approach to tracking progress and reporting outcomes.
