OMH Employee HOPE Program

Name: HOPE stands for Helping Our People Every day

A newly formed program established to help employees when they are in a time of need.

Program Details:

• Initial funding: $25,000 seed money approved by leadership team
• Ongoing sustainability: Contributions from OMH employees through payroll deductions
• Projected annual support: Just $1 per pay period, per employee would provide in excess of $15,000 in support per year
• Additional funding: Donor-directed gifts received for the Employee HOPE Program will be deposited accordingly

• Current OMH employees experiencing a time of need

How to Apply for an OMH HOPE Grant

1. Download application:
- Download and complete the Application for OMH HOPE Grant Form
- OR request form from your campus HR Representative

2. Complete application:
- Fill out all required information
- Gather any supporting documentation

3. Submit application:
- Send the completed application and supporting documentation to Steve Petitjean at the Ohio Masonic Home Foundation
- Alternative: Provide to your campus HR Representative

How to Give to the Employee HOPE Program

1. Download giving form:
- Download the Employee Giving Form from the website
- OR request form from your campus HR Representative

2. Complete and return:
- Complete the form
- Return to your HR Representative
- OR send to the Ohio Masonic Home Foundation
